Botox Touch Up: When to Get One and What to Expect

The right time for a Botox touch up is about two weeks after your treatment, once the full effect has had time to settle. At Catara Med Spa in Algonquin, IL, visible effects typically begin in 3 to 5 days and the full effect peaks at 10 to 14 days, so an area that looks uneven or still moves on day 5 can look quite different by day 14. Results vary, and your injector decides at that point whether a touch up would help.

Prefer to see it first? Watch Lily Zelaya explain why she waits 14 days before touching up Botox, then read on for the full guide.

What Is a Botox Touch Up?

A Botox touch up is a small adjustment once your treatment has settled, for an area that is still moving more than you and your injector planned. In Catara’s video, Lily describes it as something that might need “a couple of units,” not a second full treatment.

Botox and Xeomin are prescription wrinkle relaxers. They relax the small facial muscles whose repeated contractions crease the skin, so dynamic lines soften. Because the effect builds gradually, what you see in the first week is a work in progress.

Dosing units are not interchangeable between botulinum toxin products, so your plan is built around the specific product used. If a touch up makes sense, your injector plans it around the product you received at your treatment.

When Should You Get a Botox Touch Up?

Around the two week mark. Our Botox and Xeomin in Algonquin, IL page puts the full effect peak at 10 to 14 days, and that is the earliest point where you are judging a finished result rather than one that is still developing.

Lily Zelaya, a Registered Nurse and Aesthetic Injector at Catara, does not touch up at day 7. Her reasoning is simple: results on day 7 are not the same as results on day 14, and there are still about seven days left for the product to finish settling.

So if something looks off in the first week, the most useful step is usually to note it and wait. Something that looks like it needs a couple of units on day 7 may look different by day 14, as Lily explains in the video.

Planning around a wedding, a trip or photos? Because the full effect peaks at 10 to 14 days, ask your injector about booking so that peak falls before your event. If you think you may want a touch up, mention it when you book so there is time for the two week check as well.

Your Botox Timeline, Day by Day

Here is how the first two weeks typically unfold, combining the timeline on Catara’s treatment pages with what Lily describes from her own practice. Results vary from person to person.

  • Day 1: Nothing has kicked in yet. Lily notes you will still have all of your natural movement.
  • Days 3 to 5: Visible effects begin to appear as the treated muscles relax.
  • Days 3 to 7: Lily calls this the stage where things “can start to look a little weird.” You may notice one side settling before the other.
  • Days 7 to 13: The effect continues to settle, so the picture keeps changing.
  • Days 10 to 14: The full effect peaks.
  • Around day 14: Lily’s touch up assessment point, once the result has had the full two weeks.
  • Months 3 to 4: On average, the effect wears off and the treated muscles move again.

For the months after the peak, our guide to how long Botox lasts walks through the timeline month by month.

Why Botox Can Look Uneven in the First Week

An uneven brow or a line that is still moving on day 4 is one of the most common reasons clients start wondering about a touch up. Lily’s point is that this early stage is not the final result.

She explains that there is a lot of nuance in why one side can settle before the other. What she often sees, even in herself, is that one side is stronger than the other. For Lily personally, her left side takes a little longer to kick in than her right.

That is why she is clear that a side still moving, or lines that are still visible, does not automatically mean you need a touch up, especially between days 3 and 7. The same treatment can simply be arriving on a slightly different schedule on each side.

If the difference is still there once you reach the two week mark, that is exactly what the touch up assessment is for. Bring it up, and your injector will look at it with you.

Why Adding Units Too Early Can Backfire

Waiting is not just caution for its own sake. Lily explains that adding more units prematurely can often leave clients feeling very heavy, especially in the frontalis, the muscle of the forehead.

Her rule of thumb sums it up: she can always add more Botox, but she cannot take away units that have already been administered. Once product is placed, the only way back is time.

Catara’s treatment page lists drooping of an eyelid or a brow, or weakness of a muscle near the treated area, among the possible side effects. Those effects are expected to ease as the product wears off, which on average happens over 3 to 4 months. That is why Lily waits the full 14 days to touch up: it keeps her from adding product to a result that was still developing.

What Happens at a Touch Up Assessment

A touch up assessment is a check of how your treatment settled, compared with the goals you set at your consultation. Here is what it typically involves:

  • A look at movement. Your injector evaluates your muscle activity and facial expressions, the same way your original plan was built.
  • A side by side comparison. If one brow sits higher or one area still moves more, your injector checks whether the difference is real at full effect or was only a first week effect.
  • A decision, not an assumption. Sometimes a couple of units help. Sometimes the best answer is to leave a result that has settled well, or to adjust the plan at your next full treatment instead.
  • The same product. If units are added, they are planned around the product used at your treatment, since dosing units are not interchangeable.

Our Xeomin injections page describes the same habit at the end of every visit: your injector checks the treated areas before you leave, and if you need a follow-up visit, we schedule it with you.

Every plan is personal, so ask your injector at your visit how a touch up fits into your treatment. If a wrinkle relaxer is not the right fit for a concern, we will tell you and explain the better option.

One tip that helps: if you notice something in the first two weeks, jot down the day and what you saw. It gives you and your injector a clear picture of how your result developed.

When to Contact Your Injector Sooner

A touch up is about fine tuning a normal result. Some symptoms are not something to wait out until your two week check. Catara’s Botox and Xeomin page lists these:

  • Drooping of an eyelid or a brow, or weakness of a muscle near the treated area. Tell your injector, and tell them again if it does not ease as the product wears off.
  • Vision changes, such as double vision or blurred vision.
  • Trouble swallowing, speaking or breathing, generalized muscle weakness, hoarseness or loss of bladder control. Seek medical care right away if you or someone with you notices any of these.
  • Signs of an allergic reaction, such as trouble breathing, swelling or a widespread rash after an injection. Seek medical care right away.

Our treatment page notes that the effects of botulinum toxin products can spread from the area of injection, and that these symptoms have been reported hours to weeks after injection. Swallowing and breathing difficulties can be life threatening, which is why they call for urgent care, not a touch up appointment.

Touch Up vs Your Next Botox Appointment

A touch up and a repeat treatment answer two different questions. If your injector schedules a follow-up around two weeks, it is a check on the treatment you just had, and Lily describes any touch up as a couple of units, not a second full treatment. A later repeat treatment is how you keep your results once the effect wears off.

Most clients enjoy their results for 3 to 4 months on average, which is when the treated muscles start to move again and it is a good time to talk with your injector about your next visit. Results vary.

Your injector sets your maintenance schedule with you, and treatment more often than every 3 months is not part of our planning. Maintaining a regular schedule may extend longevity over time, though this varies from person to person. Ask at your visit how any follow-up fits your treatment.

At your next full treatment, tell your injector how your last result looked at two weeks and how long it held. That history helps you plan the next round together.

Botox or Xeomin: Does Touch Up Timing Change?

Not much. Xeomin, our preferred wrinkle relaxer, follows a similar first two weeks. Our Xeomin injections in Algonquin, IL page says you will usually see a change within 3 to 5 days and the full effect by 10 to 14 days.

Our guide to how long Xeomin takes to work breaks those two weeks down day by day, with day 14 as the point where the result has fully settled. That makes the two week mark a sensible time to judge either product.

Xeomin is made without accessory proteins, which makes it a good option for clients with sensitivities. Your injector recommends the product that makes sense for your goals at your consultation.

One more distinction worth knowing: wrinkle relaxers address movement lines, while fillers address volume. If your question at two weeks is really about lost volume rather than movement, your injector may talk you through dermal fillers in Algonquin, IL instead of more units.

Botox Touch Up FAQs

How long after Botox can you get a touch up?

Usually about two weeks after treatment. The full effect peaks at 10 to 14 days, and Lily waits the full 14 days before deciding whether to add units.

Is it normal for Botox to look uneven after one week?

It can be. Lily explains that one side may settle before the other between days 3 and 7, and that this is not the final result. If it is still uneven at two weeks, raise it with your injector.

Does everyone need a Botox touch up?

No. Movement or lines at day 7 do not automatically mean you need a touch up. Your injector decides at the two week assessment whether a touch up would help.

Can Botox be taken back if too much is used?

Not on demand. As Lily puts it, she can always add more but cannot take away units that have already been administered. The effect is temporary and wears off on average over 3 to 4 months.

Should I get a touch up before a big event?

Plan the treatment itself so the 10 to 14 day peak lands before your event, and leave room for a two week check if you may want one. Our Xeomin guide suggests booking at least 2 weeks ahead of a major event.
